Youth Work Permit

Work permits can be processed in person or remotely
Office hours are Mon - Fri, 7:45 am - 3:00 pm**Summer office hours are Mon-Fri, 8:30 am - 2:30 pm**
Please call 781-639-3100 ext. 20113 prior to arriving.- Download the work permit application form.
- Your employer must complete and sign page 1 of the application form
- If you are 14 or 15 years old, your doctor must sign page 2 of the application form. Skip this step if you are 16 or older
- Your parent/guardian and you must sign page 2 of the application form
- Students must sign page 2 of the application form
- Email the following items to Mrs. Loretta Benman at benman.loretta@marbleheadschools.org for your work permit to be processed remotely
- Completed work permit application form
- Photo scan of your passport or birth certificate
- Student's eye & hair color
- Home address
- City and state of birth
- Grade last completed and school attending
- Age in years and months (ex: 16 years, 3 months)
- All processed work permits will be emailed back as a pdf file with the required district signatures
- Students will need to sign the work permit. Give the work permit to the employer once the work permit is completed.
If you are not a current MHS student, but live in Marblehead - we can process your work permit. Please follow all the steps above, and also include:
- School you currently attend
- Grade level
- Home address
- Photo scan of your school ID or school transcript-
A birth certificate or current passport is needed to process the work permit. A driver's license cannot be used
